Where are you located?

Five and Ten Physiotherapy Clinic is located in a medical building at 3025 Hurontario Street, suite 203 on the second floor. The closest major intersection is Hurontario st. and Dundas (Highway Five and Highway Ten). Easy access by public transportation.

 

Where can I park?

There is plenty of free parking behind the medical building.

 

Is a doctor’s referral needed for physiotherapy?

A doctor’s referral is not required to receive physiotherapy treatment although we prefer that our patients are referred for therapy by their physician. Some extended healthcare insurance providers, MVA claims, as well as WSIB require a doctor’s referral.

 

What should I bring to my first appointment?

Please have the following with you:

OHIP card

Doctor referral

Diagnostic imaging reports (X-ray, CT scan, MRI, Ultrasound).

Post-surgical instructions/ protocol if you were provided with it by your surgeon.

If you have EHC insurance coverage, bring your insurance card with all the pertinent details such as policy and or ID number.

If you are an MVA patient, we require your claim number, policy number, the name of the insurance company, insurance branch location, the name of your adjustor if known and driver’s license for identification purposes.

If you are a WSIB patient, we require a claim number.

 

What clothing should I wear for my appointment?    

Wear loose comfortable clothing, ideally athletic attire and running shoes if possible. The therapist needs access to the body part that is of your concern.

 

How long will I be in the clinic for each session?

You should give yourself at least 60 minutes for your initial assessment and treatment and arrive at least 15 minutes early for your first appointment to complete our intake forms. The length of the subsequent treatments will vary depending on your individual needs.

 

What happens at my first appointment?

Your physiotherapist will conduct the initial assessment, inform you about the outcome, and discuss the plan of care with you. You will also receive the first treatment.

 

How many treatments will I need?

This is to be determined on the initial assessment provided by the physiotherapist.

 

Does extended health insurance cover my treatment?

Most insurance/health plans cover physiotherapy and massage treatments. Check with your insurer for specific requirements (need for a doctor’s referral, details of coverage etc.).

 

Does the clinic bill my treatment directly to my insurance company?

Yes, in many cases we are able to directly bill part or all of the treatment cost to your extended health insurance. Please check with your plan provider to confirm your coverage and whether your plan allows for online submission.
