EHC

In most of the cases, Five and Ten Physiotherapy Clinic is able to bill to your insurance company directly. Please note that every plan has different policies and restrictions. It is recommended that prior to your visit, you inquire with your provider for coverage details. For patients who wish to submit the claim themselves, we accept debit, major credit cards and cash.

WSIB

Five and Ten Physiotherapy Clinic is a registered service provider. We need to be provided with your claim number and this will allow us to charge WSIB directly for the services provided to you.

MVA

Five and Ten Physiotherapy Clinic is a FSCO licensed provider. We deal with your insurance company directly given the authorization/consent from the patient.  You will be informed about all the invoices that are being submitted pertaining to your care.

Cancellations

24 hour notice must be given to cancel or change appointments. Failure to do so will result in a regular treatment charge.
